Health examinations are required for all new students to the district and all students in pre-kindergarten, kindergarten, and grades 1, 3, 5, 7, 9, and 11. A dental health certificate is requested at the same time as required health appraisals.
Vision and hearing screenings follow New York state education law guidelines.

Medical professionals are expected to complete the form when doing physical exams for students entering a new school at any point during the year, as well as for:
- Students entering pre-Kindergarten, kindergarten, and grades 1, 3, 5,7, 9 and 11
- Students who play interscholastic sports
- Students requesting working papers
- Students who require them by their Committee on Special Education (CSE) or Committee on Pre-School Education (CPSE)
The form may be printed and completed by hand, or completed electronically by saving it to a computer, entering information into the fillable fields and saving a copy. Please bring it with you to your student’s next physical exam so it can be completed during the appointment.

When should you keep your child home from school?
A child who is sick will not be able to perform well in school and is likely to spread the illness to other children and staff. Parents must make a plan for childcare ahead of time so you will not be caught without a comforting place for your child to stay if he or she is ill.
Our district’s recommendation is that you should not send your child to school if he/she has:
- Fever in the past 24 hours
- Vomiting in the past 24 hours
- Diarrhea in the past 24 hours
- Chills
- Sore throat
- Strep Throat (must have been taking an antibiotic for at least 24 hours before returning to school)
- Bad cold, with a very runny nose or bad cough, especially if it has kept the child awake at night
Please be aware of the symptoms of COVID-19 and keep your child home if he or she shows any of the following:
- Temperature of 100 or greater
- Fever/chills
- Cough
- Loss of taste or smell
- Unexplained rash
- Fatigue/feeling tired
- Sore throat
- Shortness of breath
- Nausea/vomiting
- Pink eye
- Muscle pain
- Headaches
- Nasal congestion
- Diarrhea
If your child becomes ill at school and the school nurse feels the child is too sick to benefit from school or is contagious to other children, you will be called to come and take your child home from school. It is essential that we have a phone number where you can be contacted during the day and an emergency number in the event you cannot be reached. Please be sure that arrangements can be made to transport your child home from school (sick children are not permitted to ride the bus) and that childcare is available in case of illness. If your daytime or emergency phone number changes during the year, please notify the school immediately.
